EXCERPT FROM THE MINUTES OF THE REGULAR SESSION OF THE SANGGUNIANG BAYAN OF MAMBURAO, OCCODENTAL MINDORO HELD ON NOVEMBER 23, 2009 AT SUNNGANIANG BAYAN SESSION HALL.

MUNICIPAL ORDINANCE No. 20               
               
"An Ordinanance Adopting the Batch request entry System ( BREQS) - LGU  System of National Statistics Office (NSO) and for the purpose adjusting the fees beiing collected by this Local Government Unit of Mamburao, Occidental Mindoro "                       

"Whereas, The National Statistics Office (NSO) ventured into a program called Batch Request
Entry System which will provide off-line method of encoding request for civil registry documents and
submission  thereof to an on-line Serbilis outlet of the NSO;"               
"Whereas, by adopting the program it ill enable the LGU of Mamburao to provide an extensive service to our constituents through the office of Municipal Civil Registrar, by receiving and servicing request for Civil Registry documents registered in this Municipality as well as those that may have been registered in other places of the country. "               
               
NOW THEREFORE, BE IT ORDAINED, BY THE SANGGUNIANG BAYAN OF MAMBURAO, in its session assembled that:                


"SECTION 1. This ordinance shall be known as an ""Ordinance Adopting the Batch Request Entry System(BREQS)- LGU System of the National Statistics Office ( NSO)"               
               
               
SECTION 2. Declaration of Policy - The mUnicipal Government of Mambuao, Occidental Mindoro seeks to provide service points for the decentralized servicing of request for civil registry documents registered in this municipality as well as those that may have been registered in other places of the country and submission  thereof to an on - line serbilis outlet of the NSO.                
               
               
"SECTION 3. Purpose- The pupose of this ordinance is to receive, process and serve the request of constituents for registry documents registered in this municipality as well as those that may have been registered in other places of the country. "               
               
               
SECTION 4. Definition of terms - When used in this ordinace the following words and phrases shall mean:                
               
NSO ( National Statistics Office) - an Agency headed by the Civil Registrar - General who perform the function of the monitoring the civil registration activities in all parts of the country.               
"Local Civil Registry Office ( LCRO) - is an officer or department headed by the Local Civil Registrar who is mandated to perform civil registration function."               
               

BREQS ( Batch Request Entry System) - is a scheme where NSO authorizes a partner ti receive request for NSO issued copies and certifcation of civil registry documents from the public and issue the documents  to its clientele.                
               
               

Serbilis Outlet - NSO designated oulet whwre public can file their request fro copies and certications from NSO.                

"Civil Registry Documents - are documents containing all facts and acts concerning the civil status of person such as the Certificate of Live Birth, Certificate of Marriage, Certificate of Death, Certificate of Foundling, Court Orders/Decress, Application for Marriage License and others. "               
               
               
Certificate of Live Birth - a documens showing all the facts or information about child                
               
Certifcate of Marriage - a certificate which contains information about the groom and the bride                        
               
Certicate of Death - a document containing information about the deceased.                
               
"CENOMAR ( Certificate of No Marriage) - a document certifying that a person has not  yet contracted marriage  with anyone. "

Clients - the person or he public who wants to secure NSO copies of birth certicates, marriage certificates, death certificates and CENOMAR                
Fees - amount authorized by law to be collected for the issuance of certified copies or certifications of civil registry documents                
SECTION 5.  Aministrative provisions                
               
a. For every application filed in the Office of the Municipal Registration the applicant shall paythe amount P 80.00 as BREQS service fee at the Office of the Municipal Treasurer           
               
"b. The Municipal Civil Registrar, upon presentation of the receipt, shall then issue the certificate applied  for and accordingly recird such transactions in a logbook kept this purpose. "               
           
SECTION 6. Repealing Clause - All local laws, rules and regulations which are incosistent with this ordinance are hereby repealed, amended or modified accordingly.                
               
               
"SECTION 7. Effeftivity Clause - This ordinace shall take effect fifteen (15) days after its publication in a newpaper of local circulation and/ or posting in conspicuous places within the jurisdiction of this municipality. The ordinance shall also be forwarded to the Sangguniang Panlalawigan for its appropriate action.
"               
               
ENACTED AND  APPROVED this on November 23, 2009 at the Sangguniang Bayan Session Hall upon motion of HON. EDUARDO S. MANEJA, JR. Chairman, Committee on Finance and Appropriation duly seconded by all the members present.
